Orale Manifestation von Morbus Crohn – ein interdisziplinäres Krankheitsbild

Zusammenfassung

Sowohl Parodontalerkrankungen wie Gingivitis und Parodontitis als auch chronisch-entzündliche Darmerkrankungen (CED) wie Morbus Crohn und Colitis ulzerosa sind chronische Entzündungskrankheiten. Ihre Pathogenese wird durch ein komplexes Zusammenspiel zwischen einer dysbiotischen Mikrobiota und der entzündlichen Immunantwort des Wirts vermittelt, zusätzlich gibt es bei beiden Erkrankungen eine multifaktorielle Komponente, sodass auch genetische Faktoren und Umweltfaktoren einen Einfluss haben können. Ziel dieses Artikels soll es sein, einen kurzen Überblick über die möglichen komplexen pathogenen Interaktionen zwischen Parodontalerkrankungen und CED zu geben. Es wird angenommen, dass sich bei beiden Erkrankungen die Zusammensetzung der Mikrobiota verändert und somit auch die Entzündungsreaktion verstärkt wird. Zusätzlich soll anhand eines Fallbeispiels, das die orale Manifestation eines Morbus Crohn bei einem 12-jährigen Jungen zeigt, die Notwendigkeit der interdisziplinären Zusammenarbeit von Zahnmedizinern und Medizinern verdeutlicht werden.

Abstract

Periodontal diseases such as gingivitis and periodontitis as well as chronic inflammatory bowel diseases (IBD) such as Crohn’s disease and ulcerative colitis are chronic inflammatory diseases. Their pathogenesis is mediated by a complex interplay between a dysbiotic microbiota and the host’s inflammatory immune response; in addition, there is a multifactorial component in both diseases, so that genetic and environmental factors can also have an influence. The aim of this article is to provide a brief overview of findings on possible very complex pathogenic interactions between periodontal disease and IBD. It is assumed that the composition of the microbiota is altered in both diseases and that the inflammatory reaction is thus also intensified. In addition, a case study showing the oral manifestation of Crohn’s disease in a 12 year old boy will be used to illustrate the need for interdisciplinary cooperation between dentists and doctors.
